Island Wide Case Management Services

WHAT WE DO:  Services for Seniors, Inc. is one of several contracted vendors for the County of Hawai'i.  We provide case management to eligible kupuna through the Kupuna Care grant.  We assist elderly residents so that they can continue living at home safely for as long as possible.

WHAT WE DO:  Services for Seniors, Inc. is one of several contracted vendors for the County of Hawai'i.  We provide case management to eligible kupuna through the Kupuna Care grant.  We assist elderly residents so that they can continue living at home safely for as long as possible.  


Our staff assess your needs and abilities, develop person-centered care plans, conduct follow up visits to assess your progress and provide some durable medical equipment not covered by Medicare or Medicaid.  


Kupuna Care assists families by providing a variety of different short-term caregiving and support services such as:


* adult day health programs,

*personal care,

*heavy chore,

*light house keeping,

*transportation,

*and Caregiver Respite. 


Our Case Managers coordinate those services and then assist clients with developing a long-term care plan such as: 


*hands on assistance with completing the 

Medicaid Application,

*teaching the private pay process,

*financial management,

*sharing resources,

*and developing a natural support team.


ASSISTANCE WITH MEDICAID:  The application process can be challenging for our clients and their families.  Therefore, our Case Managers will come to your home and assist you with the paperwork every step of the way.  We also have a drop in center at our office if you would rather come to us.  


ELIGIBILITY GUIDELINES:  


 *a U.S. Citizen or legal alien, 

*Hawai'i resident, 

*60 + years of age,

*not covered by any comparable government or private home and community based services,

*not residing in a skilled nursing facility, assisted living, foster care home, or other adult residential care facility,

*and unable to perform 2 or more activities or instrumental activities of daily living.
